Congratulations and welcome to the Cizik School of Nursing! We are excited to have you join our community of scholars.
Now that you've been admitted, the next step is to complete the pre-enrollment requirements. These steps are requirements of UTHealth, the Cizik School of Nursing, and many of our clinical affiliates.You will notice in your applyUTH portal that a series of holds have been placed on your account which will restrict your ability to register until each hold is satisfied. Please note that removing HOLDS can take up to 10-14 business days, thus we recommend that you start this process immediately to avoid delays.
Note: The items do not have to be completed in the order listed. However, it is recommended that you provide proof of identify as soon as possible (instructions are listed under Key Requirements: Identity Management).
Immunizations - E*Value
Students are required to submit their SON required immunizations through the E-VALUE system AND to UT Student Health Services. Please see below for instructions on how to complete this process.
E*Value - All students are required to scan and upload the required immunizations, certifications and social security consent to release form through the E-Value system.

Immunizations - UT Student Health Services
UT Student Health Services - All students are required to submit the UT Student Health Required immunization records online through the UT Student Health Immunization system (You must wait 24-48 hours after accepting the offer of admission to access the immunization system).

Criminal Background Check
All students must successfully clear a mandatory background check no sooner than three (3) months prior to their start term.
If admitted for Fall, submit background check request after June 1st.
If admitted for Spring, submit background check request after October 1st.
If admitted for Summer, submit background check request after March 1st.
Visit the website to submit your background check request at www.precheck.com (Select "Students" from the top right corner of the page and follow the instructions.) Further instructions for requesting the background check can be found here.
Accepted applicants that have significant findings in their background check will have their cases referred to a review panel appointed by the School of Nursing. Applicants may be referred to the Texas Board of Nursing to obtain a declaratory order. Please visit the Texas Board of Nursing website for more information regarding declaratory orders. For more information regarding this requirement, please refer to policy number S1270.
Drug Screening Test
All new students must successfully clear a mandatory drug screening test no sooner than three (3) months prior to their start term.
If admitted for Fall, complete after June 1st.
If admitted for Spring, complete after October 1st.
If admitted for Summer, complete after March 1st.
Visit the website to submit your drug screening request at www.surscan.com. Once at the website click on Login/Register. The School Code is UTHSC. Once you register and pay, you will receive an email from the company with testing information. Please allow up to 2 weeks to receive the notification. If you have a Residency Hold or have been assessed out-of-state tuition and would like to have your residency status reevaluated, please complete the Core Residency Questionnaire. If you have a question regarding residency please call 713.500.3388.
The Board of Regents of The University of Texas System mandates health insurance for all students enrolled in the UT System health components. As such, each UTHealth student who cannot provide evidence of continuing coverage under another approved plan will be enrolled in the student insurance plan with Academic Health Plans and assessed a health insurance fee. The health insurance fee is $2,335.00 for the academic year 2017-2018. When registration opens you will need to complete this before registering for classes; every student must complete the health insurance certification process through his/her myUTH account. You are required to complete this each semester. Through this process, students either provide information about their own coverage or confirm their participation in Academic Health Plans. For students who attest to coverage under another approved plan, the health insurance fee will not be assessed to their student account. Planner-MSN and Post-Master Students Only
All new Master and Post Master Nursing students are required to enter their full degree plan into the Planner. You may find the instructions on completing this task HERE. Please be sure to read and follow the instructions provided as well as reference the degree plan provided by your Academic Advisor. Your Advisor will remove the hold once your degree plan has been confirmed in the Planner.
If you are a new student admitted Fall 2018 or later, you will not be able to access myUTH until 6 weeks prior to your start term.
All students are required to complete the identity vetting process by providing a government issued photo ID (e.g., drivers license, passport). This process is required to grant students access to the myUTH Network and an UTH email address for official communications. Students admitted Fall 2018 or later will have their identity management hold removed when they attend mandatory in-person orientation for their respective program.
Unable to Appear in Person- If you are unable to appear in person, then you must complete the Identity Verification Form and the Information Resources User Acknowledgement Form. These forms must be completed and notarized. The original forms must be returned to 6901 Bertner, SON846, Houston, Texas 77030. These forms may only be accepted via postal mail.
University Network Access (myUTH, Canvas, email, etc.)
Once the IDM hold is removed, contact the Help Desk (713-486-4848) to have your password reset. This is required to gain full access to the university’s network (myUTH, Canvas, email, etc.).
**Once you gain access to your UTHealth email account, it is highly recommended you check your email daily throughout the duration of your program.**
This step should be completed after you have enrolled in courses.
Once you have enrolled in courses then you may obtain your ID Badge. All students are required to have an ID Badge. You may obtain your ID Badge at the Auxiliary Services Parking Office: University Center Tower (UCT) 7000 Fannin, First Floor, Houston, Texas 77030. Monday – Friday: 8:30 a.m. to 4:00 p.m. (Students are allowed free parking for 1.5 hours at UCT with their ID Badge).
Students are required to wear their ID Badge at all times while in the SON.
ID Badge Activation
This step should be completed after you have obtained your student ID Badge.
