Rashmi Momin, DNP, BBA, MSN, APRN, FNP-BC, is an Assistant Professor in the Department of Graduate Studies at UTHealth Houston Cizik School of Nursing. Her scholarship focuses on telehealth-enabled chronic disease management, continuous glucose monitoring in Type 2 Diabetes, and competency-based graduate nursing education. Her work is grounded in over 13 years of active clinical practice as a board-certified Family Nurse Practitioner.

As founder and owner of Vital Telecare, LLC, Dr. Momin has delivered telehealth-based primary care to underserved and uninsured populations across Texas since 2020, using her practice as a platform for clinical innovation in diabetes and chronic disease management. Her contributions to teaching, scholarship, and service have been recognized through UTHealth Houston Cizik School of Nursing's Faculty Award of Excellence and Values Award.

Dr. Momin leads a focused program of scholarship at the intersection of telehealth, continuous glucose monitoring, and chronic disease management, with extensions into graduate nursing education and artificial intelligence. Her peer-reviewed work appears in the Journal of the American Association of Nurse Practitioners, the Journal of Medical Internet Research, Nurse Educator, and CIN: Computers, Informatics, Nursing, and her telehealth scholarship was featured in an invited international podium presentation at Sigma Theta Tau International. Active projects include a systematic review of CGM and telehealth in Type 2 Diabetes, a nurse practitioner-led CGM and telehealth protocol, and emerging work on AI in diabetes management. She has advanced graduate nursing education through competency-based curriculum development, including the SMOR Toolkit, a faculty-led intervention for advancing physical examination skills presented at the American Association of Colleges of Nursing (AACN) and the National Organization of Nurse Practitioner Faculties (NONPF). She serves as course lead for Advanced Physical Examination across the MSN and BSN-DNP programs and teaches throughout the Translational Science sequence. She serves as Chair-Elect of the EPIC Committee, EPIC Curriculum Integration Lead, and Faculty Development Lead for the AACN Essentials Course Mapping Initiative; mentors nine DNP students through their scholarly projects; and is a peer reviewer for the Journal of the American Association of Nurse Practitioners.

Dr. Momin's career reflects a deliberate integration of clinical practice, scholarly inquiry, and faculty leadership, translating innovations in telehealth into accessible care. Her work is driven by the belief that nursing-led innovation can expand access for those most often left behind, while preparing the next generation of nurse practitioners to lead through technology without losing the relational core of care.
