Before You Apply

- Gather your application materials: Check the webpage for your specific program of interest for a complete list of requirements. All application materials must be received by the deadline. We are unable process applications with missing or incomplete documentation.
- Know your testing code:
- Pacesetter BSN students must take the HESI A2 Entrance exam using the Department ID 198326.
- For the nurse anesthesia program, the school must be sent your GRE scores using the code 6906.
Please note that students admitted to Cizik School of Nursing must personally reside in the state of Texas before the first day of class. In addition, we are unable to sponsor students requiring F-1 visas.
How to apply
Cizik School of Nursing is transitioning the application process for some of our programs to the Nursing Centralized Application System (NCAS) platform. Therefore, the process varies depending on the program to which you are applying.
DNP and BSN-DNP Nurse Practitioner Tracks: Apply via NCAS
Use NCAS if you have either a Bachelor of Science in Nursing (BSN) or a Master of Science in Nursing (MSN) degree and are applying to any of our Doctor of Nursing Practice (DNP) tracks:
- BSN to DNP Adult/Gerontology Acute Care Nurse Practitioner
- BSN to DNP Adult/Gerontology Primary Care Nurse Practitioner
- BSN to DNP Family Nurse Practitioner
- BSN to DNP Psychiatric/Mental Health Nurse Practitioner
- DNP Nurse Executive track
- DNP Nursing Informatics track
- DNP (Post-master's Entry) Nurse Practitioner/CNS or Nurse Anesthesia
Applicants applying to these programs must submit all documents to NCAS. This includes the application, personal statement, résumé, and transcripts from each post-secondary institution attended. International applicants must submit TOEFL scores (if applicable), and individuals with degrees/coursework from outside the U.S. must submit a course-by-course evaluation of international credentials through NCAS. The NCAS system will prompt you to provide names and email addresses for three references and will automatically send them the reference form for completion.
Applicants are required to pay the NCAS application fee of $75.00 and the UTHealth supplemental form fee is $60.00. Fees are nonrefundable and nontransferable among programs.
BSN, MSN, BSN-DNP Nurse Anesthesia, and PhD Program Application Process
Use UTHealth Houston’s online application if you are applying to any of the following programs:
- Pacesetter BSN or RN to BSN program
- MSN Nursing Leadership track
- MSN Family Nurse Practitioner Track
- BSN to DNP Nurse Anesthesia track
- PhD program

- Submit the online application for admissions and pay the $60.00 nonrefundable application fee via credit/debit card.

713-500-3388 - Submit official transcripts from all post-secondary institutions attended. Electronic submission is preferred. Official transcripts may be sent directly from your post-secondary institutions to student-admissions@uth.tmc.edu. Transcripts will not be accepted if emailed from the student. Transcripts submitted on paper must be printed within six months of receipt and be in a sealed envelope from the issuing institution.
- If you have international credentials, a course-by-course evaluation from an Educational Credential Evaluator must also be submitted.
- Send your official test scores (if applicable). Please be sure to review the admissions requirements for your program to determine which, if any, tests are required.
- The GRE code for sending test scores is 6906.
- The HESI A2 Entrance Exam department ID for sending scores is 198326. - For graduate admissions, send three academic/professional references using the Cizik School of Nursing recommendation form.
Please note that students admitted to the Cizik School of Nursing must physically reside in the state of Texas before the first day of class.
Notification
Once an application is received, you will be assigned a seven-digit application ID and granted access to ApplyUTH, where you can track the status of your application materials.
Once a decision is made, ApplyUTH is updated with the application status. If you are admitted, you will receive an email and a letter in the mail. If you are denied, you will receive a letter in the mail.

Q: Can I request a change of major after I’m admitted?
A: Request to change tracks are reviewed on a case-by-case basis by the track director. Students are encouraged to conduct a thorough review of the academic options before applying for admission.

Q: What is the time limit for test scores?
A: For Pacesetter BSN students, the HESI A2 must have been taken within the past two years. For nurse anesthesia students, the GRE must have been taken within the past five years. For international students, the TOEFL must have been taken in the past two years.

Q: Can the GRE be waived for nurse anesthesia applicants?
A: No, all nurse anesthesia students are required to submit the GRE.

Q: I have submitted my official transcripts, but they are listed as incomplete in ApplyUTH.
A: The Registrar's Office updates transcript status to “receive”' only when an official, final transcript has been recorded. If you have submitted a transcript with in-progress course work, it will be listed as incomplete, but the file will be forwarded to admissions for evaluation.
