Nursing students must maintain current immunizations and screenings for specific diseases to comply with our clinical partners’ requirements. Documentation must be provided to verify required immunizations or, in some cases, positive lab reports showing you have sufficient titers in your blood to protect you against specific diseases. Additional documentation must be provided throughout your program. 

Full compliance with this process is required to register for courses during your first term and all subsequent terms. 

Important note: Review the list below closely, including the timeframes in which immunization series must be started and completed. Starting the process too soon or too late could result in course registration delays.
 

  • Cizik School of Nursing Immunization and Certification Policy

    All nursing students at Cizik School of Nursing (CSON) must satisfy UTH and CSON immunization and certification requirements prior to starting their programs and students must stay up-to-date throughout their programs. Immunization and certification requirements must be valid for each semester and cannot expire during a semester. Students who do not meet requirements prior to program start or for any subsequent semesters will not be allowed to register for courses or participate in clinical activities. 

    The following immunizations and certification are required by UTH and CSON prior to registration for courses.

    TB screening, along with a TB risk assessment questionnaire, must be done within the past 6 months, even for those who received the BCG vaccine as a child. If the IGRA test is positive, a chest x-ray documenting no active tuberculosis must be submitted. The Certificate of Immunizations is available on the Student Health website.

    • You may choose to take the AHA-BLS through Cizik School of Nursing’s Simulation and Clinical Performance Laboratory.
  • COVID-19 vaccination information

    At this time, the COVID-19 vaccination is not required to enroll at UTHealth Houston but is strongly encouraged. However, most clinical sites require vaccination and some require a booster to participate in clinical activities. Students who choose not to receive the COVID-19 vaccination or booster should contact UTHealth Houston’s University Relations and Equal Opportunity team at [email protected] to request an exemption.

  • Exceptions permitted by law

    1) Proof that a health care provider has determined that vaccination would pose a health risk to the student, or 

    2) Submission of the Exemption Form issued by the Texas Department of State Health Services. If you submit this form in lieu of completing required immunizations, you may register for classes. However, clinical affiliates may not allow you in clinical settings without meeting all immunization/screening/health requirements. 


Uploading your documentation

You must upload your immunization documentation twice – once in myUTH to meet university requirements, and separately for Cizik School of Nursing. The school uses this documentation to communicate compliance with clinical facilities. 

  • myUTH:You will have access to the myUTH Immunization Portal 48 hours after accepting your offer of admission. This data is maintained by UTHealth Student Health Services.
  • School of Nursing documentation: Watch for an email from "Cizik School of Nursing <notifications.exxat.com>" to your UTHealth Houston email address approximately one week after accepting your offer of admission and releasing your IDM hold with instructions on how to access the EXXAT portal to submit your immunization documentation for the school. 

Note for current students: If you initially submitted immunization documentation via the E*Value portal when you first enrolled at Cizik School of Nursing, you will continue to use this portal until you graduate. However, most recent cohorts were admitted using the EXXAT system and will not use E*Value. 

The documentation collected via EXXAT and E*Value are maintained by the Cizik School of Nursing Compliance team. 



NAD Hold

NAD stands for “Nursing Admissions Hold.” If you see this code, check E*Value or EXXAT. Review your compliance immunization and certifications have been uploaded. If you have any questions or concerns, please call 713-500-4325.

RIM HOLD

And RIM code means your documentation is missing or incomplete in myUTH. If you believe you have uploaded the correct documents and still encounter a RIM hold, contact Student Health Services.
