Following this checklist and timeline will keep you on track to begin your nursing education at Cizik School of Nursing at UTHealth Houston.
A step-by-step guide to get you started

Within 10 days of accepting your offer of admission
- Complete the Identification Management (IDM) process for network access verification.
- Complete the Information Resources User Acknowledgement Form (also called the IT Verification form).

2-3 days after IDM process is complete
- Activate your student email account by contacting the UTHealth Student Help Desk at 713-486-4848. Many important communications will be sent to you only through your UTHealth email address.

Up to three months before class begins
- Criminal background check and drug screening: These are required by UTHealth and our clinical partners. If not completed on time, students must repeat these checks, incurring additional expense. You will be able to order your background check two to three months before classes start.
- Texas Board of Nursing background check (Pacesetter BSN students only): This separate background/fingerprint check is required prior to enrollment to ensure eligibility to take the NCLEX exam for licensure. Watch for an email from the IdentoGo with instructions and required deadlines for submission.
These process should not be started before following dates:- Fall Term – June 1
- Spring Term – October 1
- Summer Term – March 1
Without completing these critical steps, you will not be able to access necessary information systems or receive your UTHealth email address. Read these instructions and follow them carefully. -
Before registration (which begins 6-7 weeks before the semester starts)
- Send final transcripts: Check your admissions letter to determine whether you need to send official, final transcripts showing any outstanding coursework completed after your offer of admission.
- Health insurance certification: Coverage is required for all UTHealth Houston students. Log in to your MyUTH account to verify your existing coverage or confirm participation in the UTHealth Houston insurance plan.
- Core Residency Questionnaire: You must complete this questionnaire to determine whether you are eligible for in-state tuition.
- Basic Life Support certification: You must complete the American Heart Association Basic Life Support (AHA-BLS) for the Health Care Provider course. You may choose to take the AHA-BLS through Cizik School of Nursing’s Simulation and Clinical Performance Laboratory.
- Degree Plans: MSN, DNP, and BSN to DNP students must upload a degree plan in MyUTH. Please see the Graduate Students Planner Guide for details.
- Canvas access: Make sure you can access Canvas, the online course management system for all classes at Cizik School of nursing. If you have any problems, call the Student Help Desk at 713-500-4848.

At least 30 days before semester starts
- Immunization information: Provide your required immunization documents and certification information in the E*Value or EXXAT system.
- Criminal background check and drug screening: Reports must be received by Cizik School of Nursing.
- Completion of Texas Board of Nursing background check (Pacesetter BSN students only).

15 days before semester starts
- ID badge: Visit the ID badge office at the University Center Tower (UCT) building, 7000 Fannin, Suite 1.070Q, and present government-issued identification to get your badge, then complete the ID Badge Activation Form so that you can enter the Cizik School of Nursing building.
- Orientation videos and required readings - Complete these tasks and submit the acknowledgement form.
